Coding Tip: D4355 Full mouth debridement to enable a comprehensive evaluation and diagnosis on a subsequent visit, was revised in 2018 to clarify that full mouth debridement is performed to enable a comprehensive oral evaluation on a separate visit on another date. Meaning you cannot bill the D4355 at the same appointment as the D0150 or the D0180. Appoint the patient 10-14 days after the D4355. The patient has gross plaque and heavy calculous that must be removed to be able to see the teeth for the evaluation.
