D4346 Scaling in Presence of Generalized Moderate or Severe Gingival Inflammation–Full Mouth, after Oral Evaluation

Never use this code as a “difficult prophy.”  D4346 does not relate to the amount of time or effort to complete the procedure.  D4346 is a therapeutic procedure that can be performed on the same day as an oral evaluation.  It is performed to address swollen, red, bleeding gums that represent gingivitis localized without loss of attachment or bone loss(that is characterized in scaling and root planing)  A D1110 can be performed about 2-4 weeks after the D4346. Insurance policies will vary on how this code is paid. Some may include it in frequency for a prophy some may have a separate allowance for it.  Provide radiographs and periodontal charting with a narrative including “gingival inflammation and BOP generalized”
